• An Alberta Service Centre/Canada-Alberta Service Centre.
• Apprentices may also pick up forms from an Apprenticeship and Industry Training Office.

Depending on your situation, there are additional forms that need to be completed when you submit your application or when your circumstances change. If you prefer, you may complete these forms online, print them, sign them, and submit them with your application or with a Change in Circumstances form.
